KENTUCKY PUBLIC SCHOOL SYSTEM
Registration
Children must be at least five years of age by October 1 of the enrollment year in order to register for kindergarten in Kentucky . Parents must provide an official copy of the student's birth certificate and the child's social security card. In order to comply with Kentucky health laws and regulations, all new registrants must bring a physician-signed immunization certificate and proof of a physical exam. Students who transfer from out-of-state districts are required to comply with the state's immunization laws. Registration is held at the school the child will be attending, although it is not necessary that the child be present at registration.

Each child must provide an immunization certificate that verifies proper doses of all age-appropriate vaccines. All school physical exams must be completed and signed by a physician no more than one year prior to the students' first-time enrollment. First-time enrollees must also have an eye exam. No grace period is allowed for compliance on requirements for health records.

INDIANA PUBLIC SCHOOL SYSTEM
Registration
Children must be at least five years of age by July 1 of the enrollment year in order to register for kindergarten. First graders must be at least six years of age by July 1 of the enrollment year. Some school districts offer early registration of students who do not meet the age requirement; check with your local district for further information. Legal, certified birth certificates as well as official immunization records are required for all new students. These documents should be furnished at registration. Also required are proof of residency and a current address in the form of utility bills, rent receipts, or mortgage papers; the student's social security number; guardianship papers if applicable; and records from the previous school or a registrar contact.

Required health documents may include dental or physical exams, depending upon the district regulations, and up-to-date immunizations. According to Indiana law, the following immunizations are required in proper doses and intervals: Diphtheria-pertussis-tetanus (DPT), polio vaccine (OPV or IPV), measles-mumps-rubella (MMR), Hepatitis B (HBV), and a TB skin test. New students receive a grace period of 30 days to comply with all requirements, although newcomers are urged to register prior to the opening of school.
